Cancellation Policy: Foundations Physical Therapy and Rehabilitation
Cancellation Period:
Cancellations must be made at least 24 hours before the scheduled appointment to avoid any fees.
For Monday appointments, cancellations must be made by the end of the day on Friday to avoid any fees.
Cancellations can only be performed via the online portal 72 or more hours in advance. Cancellations within 72 hours of appointment must be made by calling the clinic main line at 1-709-786-1491.
Fees or Penalties:
Cancellations made within 24 hours of the scheduled appointment will result in a $35 cancellation fee.
No-Show Policy:
In the event of a no-show, Foundations Physical Therapy and Rehabilitation reserves the right to charge a no-show fee. The amount of the fee is the full charge of the scheduled service and will be applied to the client's account or invoiced separately.
Exceptions:
Exceptions to the cancellation policy may be considered in cases of documented emergencies, unforeseen events, or other justifiable circumstances. Exceptions are offered at the sole discretion of Foundations Physical Therapy and Rehabilitation
Communication Method:
Cancellations must be communicated via phone to the main office at 709-786-1491. A voicemail may be left indicating the cancellation but please include your full name, number and the time of your appointment. This excludes appointments cancelled via the online portal 72 or more hours in advance.
Additional Notes:
All fees or penalties are the responsibility of the client. Fees or penalties are not covered by insurance companies including auto insurance claims or workplace injury claims.
Foundations Physical Therapy and Rehabilitation reserves the right to change and update this policy.
